About the Role

Reporting to Behavioral Health Leadership, the Behavioral Health Social Worker is an important member of our collaborative care team. You’ll work directly with patients as well as side by side with primary care providers and health advocates (HAs) crafting, facilitating, and maintaining care plans tailored to patients’ needs and goals. The social worker will work closely with these teams to conduct care coordination and biopsychosocial assessments to identify barriers to seeking appropriate medical and behavioral health care. As a subject matter expert within the field of behavioral health, you will be the primary liaison with external organizations in the community to facilitate adherence to the care plan and will be responsible for educating patients/family caregivers on how to navigate the healthcare system.

This is a temporary assignment as we get this program off the ground in the Syracuse area.  We are open to folks who are willing to commute to the Syracuse area and/or temporary housing in the area to support the work over the next few months as we work to build our services into a new community to create some real impact.  This is a chance to really dig in and get boots on the ground to support a community of folks who usually lack access.   This could be a great way to hands on practice experience and own patient outcomes.

Here’s what you’ll do:

  • Consult directly with patients (sometimes virtually, sometimes in person in the community or in patients’ homes), understanding who they are as people, identifying their social needs and matching them to relevant resources
  • Collaborate closely with a team to craft personalized care plans in line with each patient’s needs and goals
  • Provide support and guidance to an interdisciplinary care team
  • Conduct psychosocial evaluations (including mental status exams), translate clinical impressions to care plan goals, and provide crisis intervention provide short-term evidenced based therapy, as needed
  • Identify and foster meaningful relationships with organizations in the community

Travel Requirements: This is primarily for field-based patient care and support for patients with 60% (2-3 days) of the time spent in the field seeing patients in and around Syracuse, NY and surrounding counties. The rest of the time will be remote work. You will be required to commute to patient visits through personal transportation or with our team in our mobile unit.

Compensation:  Hourly rate calculating to $60,000-$85,000 annualized salary + temporary housing/mileage

We would love to hear from you if you have the following or equivalent experience:

  • LMSW or LCSW preferred
  • 1 to 3 years post-master's experience working with the behavioral health population/within a behavioral health or integrated primary care setting, has conducted community-based work or field-based care
  • Able to asses for and intervene with SMI and SUDs
  • Experience conducting psychosocial assessments and creating plans of care
  • Willing to commute from NYC or the surrounding areas to visit our population of patients in and around the Syracuse region. (We are open to providing temporary housing for the right candidate)
